The result is blood shunting from the high-pressure system (systemic circulation) to the low-pressure system (pulmonary circulation). Often large defects<\/td><\/tr><\/tbody><\/table><\/div><div><table><tbody><tr data-start=\"1422\" data-end=\"1617\"><td data-start=\"1422\" data-end=\"1465\" data-col-size=\"md\">Muscular type \u2013 inlet septum (inlet VSD)<\/td><td data-start=\"1465\" data-end=\"1617\" data-col-size=\"xl\">Defects located directly beneath the atrioventricular valves, without a tendency for spontaneous closure. Included among endocardial cushion defects<\/td><\/tr><\/tbody><\/table><\/div><div><table><tbody><tr data-start=\"1618\" data-end=\"1885\"><td data-start=\"1618\" data-end=\"1669\" data-col-size=\"md\">Muscular type \u2013 outlet septum (malalignment VSD)<\/td><td data-start=\"1669\" data-end=\"1885\" data-col-size=\"xl\">Large defects between the outlet septum and the remaining muscular septum due to failed alignment and connection. Severe congenital heart diseases such as Tetralogy of Fallot are characterized by this type of VSD<\/td><\/tr><\/tbody><\/table><p>\u00a0<\/p><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-17aff32 e-flex e-con-boxed e-con e-parent\" data-id=\"17aff32\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-d267388 elementor-widget elementor-widget-text-editor\" data-id=\"d267388\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h2 class=\"PDq2pG_selectionAnchorContainer\" data-start=\"1892\" data-end=\"1935\" data-section-id=\"u999l5\">Hemodynamics and Clinical Manifestations<\/h2><h3 data-start=\"1937\" data-end=\"1953\" data-section-id=\"x4j1ov\">Hemodynamics<\/h3><p data-start=\"1955\" data-end=\"2304\">The presence of an intra- or extracardiac communication allows part of the blood flow to deviate from its normal course and pass through the defect into the opposite circulation. The direction and magnitude of the shunt depend on both the size of the defect and the pressure difference between the two circulations at the level of the communication.<\/p><p data-start=\"2306\" data-end=\"2684\">Since systemic circulation has higher pressures than pulmonary circulation, blood flow through the defect is directed from the systemic to the pulmonary circulation (left-to-right shunt). If the defect is small to moderate, maintaining the pressure difference between the circulations, the amount of blood flow through the defect depends mainly on the size of the communication.<\/p><p data-start=\"2686\" data-end=\"3001\">The left-to-right shunt reduces systemic cardiac output and increases pulmonary blood flow. Oxygenated blood from the left chambers (or aorta) escapes into the right chambers (or pulmonary artery), recirculating through the lungs without contributing to systemic oxygenation, thereby increasing pulmonary perfusion.<\/p><p data-start=\"3003\" data-end=\"3330\">Small defects are termed restrictive because they significantly limit shunting between circulations. Large defects are nonrestrictive and minimally impede blood flow through the communication. In this case, the pressure difference between the two circulations decreases or pressures equalize, leading to pulmonary hypertension.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-82bc489 e-flex e-con-boxed e-con e-parent\" data-id=\"82bc489\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-247c75b elementor-widget elementor-widget-text-editor\" data-id=\"247c75b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h3 class=\"PDq2pG_selectionAnchorContainer\" data-start=\"3332\" data-end=\"3359\" data-section-id=\"1ozq4g8\">Clinical Manifestations<\/h3><p data-start=\"3361\" data-end=\"3652\">The common clinical manifestations depend on the extent of the left-to-right shunt. When the shunt is limited, patients are asymptomatic. However, extensive left-to-right shunting leads to reduced systemic output and pulmonary recirculation, resulting in signs and symptoms of heart failure:<\/p><ul data-start=\"3653\" data-end=\"3761\"><li data-start=\"3653\" data-end=\"3666\" data-section-id=\"1lwe42b\">Tachycardia<\/li><li data-start=\"3667\" data-end=\"3681\" data-section-id=\"1g08qgb\">Easy fatigue<\/li><li data-start=\"3682\" data-end=\"3693\" data-section-id=\"16gmulx\">Tachypnea<\/li><li data-start=\"3694\" data-end=\"3708\" data-section-id=\"1we70no\">Hepatomegaly<\/li><li data-start=\"3709\" data-end=\"3727\" data-section-id=\"1kau9yj\">Poor weight gain<\/li><li data-start=\"3728\" data-end=\"3761\" data-section-id=\"1gls38\">Frequent respiratory infections<\/li><\/ul><p data-start=\"3763\" data-end=\"3933\">Depending on the type of defect, more specific clinical findings may exist, such as the character and location of heart murmurs, splitting of the second heart sound, etc.<\/p><h3 data-start=\"3935\" data-end=\"3948\" data-section-id=\"znindu\">Frequency<\/h3><p data-start=\"3950\" data-end=\"4209\">Ventricular septal defect (VSD) is the most common congenital heart disease, occurring in approximately 50% of patients with congenital heart disease. It may occur in isolation (20% of congenital heart diseases) or in association with other cardiac anomalies.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-050e82f e-flex e-con-boxed e-con e-parent\" data-id=\"050e82f\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-5f6e516 elementor-widget elementor-widget-text-editor\" data-id=\"5f6e516\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<h2 class=\"PDq2pG_selectionAnchorContainer\" data-start=\"4216\" data-end=\"4229\" data-section-id=\"hk2xbs\">Ultrasound<\/h2><p data-start=\"4231\" data-end=\"4455\">Ultrasound diagnosis of perimembranous ventricular septal defects is more difficult because of their location, and they must be distinguished from atrioventricular septal defects by the presence of the primary atrial septum.<\/p><p data-start=\"4457\" data-end=\"4737\"><a href=\"https:\/\/amzn-fmf-bucket.s3.eu-west-2.amazonaws.com\/course\/20251203\/107_1764739421604.mp4\">Perimembranous defects<\/a>(video)\u00a0 may be associated with chromosomal abnormalities and anomalies in other organ systems. Therefore, detailed fetal anatomical evaluation and karyotyping are necessary. Therefore, if there is no significant hemodynamic burden, or if it is adequately controlled with medication, careful observation is appropriate.<\/p><p data-start=\"5098\" data-end=\"5208\">Treatment is usually surgical, although in selected cases closure may be achieved with a transcatheter device.<\/p><p data-start=\"5210\" data-end=\"5288\">Management is individualized; however, the following general principles apply:<\/p><ul data-start=\"5289\" data-end=\"5693\"><li data-section-id=\"1nbqo36\" data-start=\"5289\" data-end=\"5470\"><strong>In infants younger than six months<\/strong> (earlier in trisomy 21), closure is recommended if heart failure is not adequately controlled medically or if pulmonary hypertension is present.<\/li><li data-section-id=\"123hwtt\" data-start=\"5471\" data-end=\"5563\"><strong>At one year of age<\/strong>, closure is recommended if the shunt is large (Qp:Qs &gt;2:1) or pulmonary hypertension exists.<\/li><\/ul><p><em>Qp = pulmonary blood flow \u2014 blood flowing through the lungs Qs = systemic blood flow \u2014 blood flowing through the rest of the body) <\/em>or pulmonary hypertension exists.<\/p><ul data-start=\"5289\" data-end=\"5693\"><li data-section-id=\"p52fxl\" data-start=\"5564\" data-end=\"5693\"><strong>Later in life,<\/strong> closure is generally recommended in the presence of left ventricular dilation despite normal pulmonary pressure.<\/li><\/ul><p data-start=\"5695\" data-end=\"5821\">Patients may also be closely monitored, with intervention at the first signs of heart failure or increased pulmonary pressure.<\/p><p data-start=\"5823\" data-end=\"6051\">Blood flow through the defect (usually membranous) may affect the aortic valve cusps, causing cusp prolapse and aortic regurgitation.
